If you have serious scarring from previous bouts with acne, there are some things you can do.

One alternative is laser resurfacing, which can be done in the doctor’s office. This therapy removes the damaged top layer of the skin and tightens the middle layer, leaving skin smoother. This can take from a couple of minutes to an hour. It usually takes between 3 and 10 days for the skin to heal completely.

Another way to treat acne scars is dermabrasion, which uses a rotating wire brush or spinning diamond instrument to wear down the surface of the skin. As the skin heals, a new, smoother layer replaces the scarred skin. It may take a bit longer for skin to heal using dermabrasion generally between 10 days and 3 weeks.

The newest form of treatment for acne scars is called fractional laser therapy. This type of treatment works at a deeper level than laser resurfacing or dermabrasion, Because fractional laser therapy doesn’t wound the top layer of the skin, and the healing time is shorter. The only problem is that this treatment is very expensive.

Autologous fat transfer. Fat is taken from another part of your body and injected into your skin. The fat is injected under the surface of the skin to lift up depressed acne scars. This method of autologous (from your own body) fat transfer is usually used to correct deep outline defects caused by scarring from nodulocystic acne. Because the fat is reabsorbed into the skin over a period of 6 to 18 months, the procedure usually must be repeated. Longer lasting results may be achieved with multiple fat-transfer procedures.

Dermabrasion. This is thought to be the most effective treatment for acne scars. Under local anesthetic, a high-speed brush used to remove surface skin and alter the contour of acne scars. Superficial scars may be removed in general, and deeper scars may be reduced in depth. Dermabrasion does not work for all kinds of scars; for example, it might make ice-pick acne scars more noticeable if the scars are wider under the skin than at the surface. In darker-skinned people, dermabrasion may cause changes in pigmentation that involves additional treatment.

Collagen injection. Collagen, a normal substance of the body, is injected beneath the skin to “stretch” and “fill out” certain types of superficial and deep soft acne scars. Collagen treatment usually does not work as well for ice-pick scars and keloids. Collagen derived from cows or other non-human sources cannot be used in people with autoimmune diseases. Human collagen or fascia is helpful for those allergic to cow-derived collagen. Cosmetic benefit from collagen injection usually lasts 3 to 6 months.

When a tissue suffers some kind of damage, the body rushes to repair the injury site. The life history of acne scars also is not well understood. Some people bear their acne scars for a lifetime with little change in the scars, but in other people the skin undergoes some degree of remodeling and acne scars diminish in size.

Most of the time, an acne scar will appear after nodular cystic acne. Acne appears either due to problems in the sebum canals that interrupt the flow of sebum to the surface of the skin or to the blockage of excess sebum by an outside material like dirt or bacteria. The body innately sends help to the blockage which ends up clogging the pore further, resulting in the nodular cyst you see in the mirror. The final phase is that people don’t like the breakout and proceed to pop their acne, which only pushes the infection deeper into the skin, resulting in a more severe inflammation and ultimately an acne scar.
